It doesn't happen to me normally, but when that does happen, I hit command tab to change the active app. That generally brings back my pointer.

Things are certainly pointing to "Flash" issues.
Well... I tried the proposed solution "delete the ~/Library/Caches/com.apple.Safari file" and didn't work. So I went back to Adobe's website and downloaded the Adobe Flash Player and reinstalled again. All good now.

I think this is a problem with spaces/expose'. I have struggled with the problem for about 2 years (10.5 and 10.6) and some months back found a work around that always works and does not require waiting forever, or rebooting my machine. This may only apply to MBPs. But since I have 3 of them afflicted (my XServe is not) I have been able to verify it across an older 15" MBP (Intel), and my Unibody MBP 15" and 17".
1 - Do the 4 finger swipe on the track pad. (Or however you initiate the switch applications widget)
2 - Select any app via arrow keys.
2 - Mouse Pointer reappears.

I had this problem and it usually occurred immediately after I'd performed a keyboard shortcut. I unchecked the ignore accidental trackpad input in my system prefs and the problem went away.
